Why four gates, not one review
Most "AI with human oversight" means a person skims an output before it ships. That catches typos, not systemic errors. Accomply separates validation from review from sign-off from evidence because each catches a different failure mode: automated checks catch what machines are reliable at catching - totals, schema, cross-references, known hallucination patterns. Domain review catches what requires professional judgement - is this the right treatment, does this match the source intent.
